Welcome to Onyx Glow IV. These Terms & Conditions govern your use of our website, booking platform, forms, communications, memberships, and services. By accessing our website, booking an appointment, purchasing a service, enrolling in a membership, submitting a form, or communicating with us, you agree to these Terms & Conditions.

Please read these Terms carefully before using our services.

1. Services Provided

Onyx Glow IV provides wellness-focused services that may include IV hydration therapy, vitamin injections, wellness boosters, concierge services, event services, and related wellness offerings.

All services are subject to medical screening, provider review, and clinical approval. Not every client will qualify for every treatment.

Onyx Glow IV reserves the right to refuse, modify, delay, or discontinue services if a provider determines that treatment is not appropriate or safe.

2. Not a Substitute for Emergency or Primary Medical Care

Our services are intended for wellness and supportive care purposes only. They are not a substitute for emergency care, primary care, urgent care, diagnosis, or treatment of a medical condition.

If you are experiencing a medical emergency, including but not limited to chest pain, shortness of breath, severe allergic reaction, fainting, stroke-like symptoms, severe dehydration, severe abdominal pain, confusion, or any life-threatening symptoms, call 911 or seek emergency medical care immediately.

3. Medical Screening and Eligibility

Before receiving services, you may be required to complete an intake form, medical history, medication review, allergy review, consent form, and/or brief clinical screening.

You agree to provide accurate and complete information regarding your health history, allergies, medications, pregnancy status, medical conditions, and current symptoms.

Failure to provide accurate information may increase the risk of adverse reactions and may result in denial or cancellation of services.

4. Provider Approval Required

All treatments are subject to approval by a licensed medical provider or authorized clinical team member. Provider approval may be based on your health history, symptoms, vital signs, medications, allergies, contraindications, treatment goals, or other safety considerations.

Payment or booking confirmation does not guarantee that a service will be provided.

5. Consent to Treatment

By receiving services from Onyx Glow IV, you acknowledge that you have been given the opportunity to ask questions and understand the nature of the service, potential benefits, risks, alternatives, and limitations.

Potential risks may include, but are not limited to:

  • Bruising, pain, swelling, or bleeding at the injection/IV site

  • Infection

  • Dizziness, fainting, or lightheadedness

  • Allergic reaction

  • Nausea or discomfort

  • Fluid overload or electrolyte imbalance

  • Medication-related side effects

  • Failure to achieve desired results

  • Need for medical evaluation or emergency care

You may refuse or stop treatment at any time.

6. Booking and Appointments

Appointments may be booked through our website, booking platform, phone, text, email, or other approved channels.

You are responsible for providing accurate appointment details, including location, contact information, and service requests.

For concierge or mobile services, you must ensure that the treatment location is safe, clean, accessible, and appropriate for services.

Onyx Glow IV may reschedule, delay, or cancel appointments due to provider availability, unsafe conditions, incomplete intake forms, weather, emergency situations, or clinical concerns.

7. Cancellations, Rescheduling, and No-Shows

We require at least 24 hours’ notice for cancellations or rescheduling unless otherwise stated.

Late cancellations, missed appointments, or failure to complete required forms may result in a cancellation fee, forfeiture of deposit, or loss of service credit.

Suggested policy language:

  • Cancellations with 24+ hours’ notice: eligible for rescheduling

  • Cancellations with less than 24 hours’ notice: may be charged a cancellation fee

  • No-shows: may be charged the full service amount or lose the service credit

  • Late arrivals: may shorten the appointment or require rescheduling

Onyx Glow IV reserves the right to waive or enforce cancellation policies at its discretion.

8. Payments

Payment may be required at the time of booking, before treatment, or at the time of service.

Accepted payment methods may include credit card, debit card, electronic payment, invoice, or other approved methods.

You agree to pay all fees associated with the services, memberships, add-ons, travel fees, concierge fees, cancellation fees, and applicable taxes.

Prices are subject to change at any time.

9. Refund Policy

Due to the nature of medical and wellness services, completed services are generally non-refundable.

Refunds may not be issued for:

  • Completed treatments

  • Missed appointments

  • Late cancellations

  • Unused monthly membership credits

  • Services denied due to inaccurate intake information

  • Failure to follow pre-treatment or post-treatment recommendations

If a provider determines that you are not medically eligible for treatment after screening, Onyx Glow IV may offer a refund, credit, or rescheduling option at its discretion.

10. Membership Terms

Memberships are billed monthly unless otherwise stated.

By enrolling in a membership, you authorize Onyx Glow IV or its payment processor to charge your selected payment method on a recurring basis.

Membership benefits may include monthly IV credits, injection credits, discounts, priority booking, concierge perks, boosters, or other member benefits, depending on the selected plan.

Membership terms:

  • Memberships renew automatically each billing cycle

  • Unused sessions or credits reset at the end of each billing cycle unless otherwise stated

  • Membership credits are non-transferable

  • Membership credits have no cash value

  • Membership benefits cannot be combined with certain promotions unless approved

  • Additional services, upgrades, concierge fees, travel fees, or add-ons may cost extra

  • Services remain subject to medical screening and provider approval

11. Membership Cancellations and Changes

Members may cancel, pause, or upgrade/downgrade their membership according to Onyx Glow IV’s current membership policies.

Suggested policy:

Membership cancellation requests must be submitted at least 7 days before the next billing date to avoid the next monthly charge.

Onyx Glow IV may require cancellation requests to be submitted in writing by email, online form, or another approved method.

Membership changes may take effect during the next billing cycle unless otherwise approved.

12. Promotions, Discounts, and Gift Cards

Promotions, discounts, coupons, gift cards, and special offers may be subject to separate terms.

Onyx Glow IV reserves the right to modify, limit, or discontinue promotions at any time.

Promotions may not be combined unless stated otherwise.

Gift cards or promotional credits may not be redeemable for cash.

13. Event and Group Services

Event, pop-up, corporate, bridal, wellness, or group services may require deposits, minimum participant counts, travel fees, setup requirements, and separate agreements.

Event deposits may be non-refundable unless otherwise stated in writing.

Final pricing may depend on location, number of participants, service type, staffing needs, supplies, and event duration.

All participants must complete required intake and consent forms and be medically approved before receiving services.

14. Concierge and Mobile Services

For mobile or concierge appointments, clients are responsible for providing a safe and appropriate location.

Onyx Glow IV may refuse to provide services in locations that are unsafe, unsanitary, inaccessible, excessively crowded, or not appropriate for medical treatment.

Examples include areas with poor lighting, lack of privacy, unsafe pets, intoxicated or disruptive individuals, or conditions that prevent safe clinical care.

Additional travel or concierge fees may apply.

15. Client Conduct

Clients agree to treat Onyx Glow IV staff, providers, and partners with respect.

Onyx Glow IV may refuse or discontinue service for inappropriate conduct, harassment, unsafe behavior, intoxication, threatening behavior, incomplete information, or failure to follow provider instructions.

16. Results Not Guaranteed

Individual results vary. Onyx Glow IV does not guarantee specific outcomes, symptom relief, performance improvement, weight loss, beauty results, recovery time, or wellness benefits.

Statements about services are for general informational purposes and should not be interpreted as guarantees or medical claims.

17. Website Content and Medical Disclaimer

Content on our website, social media, marketing materials, emails, texts, or other communications is for general informational and wellness purposes only.

It does not replace professional medical advice, diagnosis, or treatment.

You should consult your primary care provider or appropriate healthcare professional regarding medical concerns.

18. Privacy and Health Information

Your use of our services is also governed by our Privacy Policy.

We may collect personal and health-related information to provide services, complete medical screening, schedule appointments, process payments, and communicate with you.

Health-related information may be subject to applicable healthcare privacy laws depending on how services are structured and delivered. HIPAA protects medical records and other individually identifiable health information when handled by covered entities and certain business associates.

19. SMS/Text Messaging Terms

By providing your mobile phone number and opting into SMS communications through our website forms, you consent to receive text messages from Onyx Glow IV related to:

  • Appointment scheduling

  • Appointment confirmations and reminders

  • Intake coordination

  • Customer support communications

  • Service-related updates

We are not responsible for the privacy practices, security, errors, downtime, or terms of third-party platforms.

You may be subject to the terms and policies of those third-party providers.

21. Intellectual Property

All website content, branding, logos, text, images, graphics, service names, designs, and materials are owned by or licensed to Onyx Glow IV unless otherwise stated.

You agree to indemnify and hold harmless Onyx Glow IV, its owners, providers, staff, contractors, affiliates, and partners from claims, damages, liabilities, costs, or expenses arising from:

  • Your use of our services

  • Inaccurate or incomplete information you provide

  • Your violation of these Terms

  • Your failure to follow provider instructions

  • Unsafe or inappropriate service location conditions

  • Misuse of website content or communications

25. Changes to Services or Terms

Onyx Glow IV may update, modify, suspend, or discontinue services, pricing, memberships, policies, or these Terms at any time.
